The effective and efficient management of population health means collaborating with community entities and includes patients, families, health care organizations, community organizations, and associations. In addition, a biopsychosocial approach to care includes incorporating evidence-based practice and standards as well as the regulatory and national health care initiatives as considerations in care planning.
